Quality of Life Destroyed By Chronic Daily Headache according to Cephalgia article. Neuromuscular Dentistry can improve Quality of Life

A total of 34 studies were reviewed in this paper. Chronic Daily Headache (CDH) and Chronic Daily Headache with Medication Overuse (MOH) consistently created a lower quality of life. The Cephagia Article "Quality of life impairment, disability and economic burden associated with chronic daily headache, focusing on chronic migraine with or without medication overuse: A systematic review." include 25 studies of patients and 9 studies of the general population.

I strongly recommend Neuromuscular Intervention as soon as possible when chronic headaches begin. These studies clarify the importance of utilizing a diagnostic neuromuscular orthotic early in the course of the problem. Almost every study done on headache treatment with a neuromuscular diagnostic orthotic shows at least a 50-80% improvement with NMD and frequently far superior results than medication.

Chronic Daily Headache frequently responds extremely well to Neuromuscular Dentistry but unfortunately a diagnostic orthotic is rarely offered to patients in pain centers and neurology offices. The biggest complaint about Neuromuscular Dentistry is that it can be expensive and time consuming when compared to writing a perscription. Long term savings and improvement in quality of life are essential considerations that must be taken into consideration. Insurance companies frquently are uncooperative using sneaky contract language to deny medically necessary treatment. One of the most common and unquetionably fraudulant techniques is to call all headaches and migraines treated by a dentist TMJ or TMD and then place an artificially low coverage maximum on that treatment. The article clearly states "Chronic Daily Headache was consistently associated with greater disability and productivity loss, more consultations, more or longer hospitalizations and higher direct costs than Episodic Headache, it is essential."

The principal conclusions of this review were"the findings of this review underline the detriment to Quality of Life and the disabling nature of CDH, and in particular CM and CDH with MOH, and negative impact on workplace productivity compared to other types of headache."

Reducing medication use and overuse is one of the real advantages of Neuromuscular Dental treatment of Chronic Migaine and Chronic Daily Headache. The side effects of massive drug use in headache and migraine often worsen the problem.

Prior to treating migraines, Tension -type headaches sinus headaches, and chronic daily headaches with dangerous medications it is logical to utilize a Dignostic Neuromuscular Orthotic, if relief is obtained the Medication Overuse is diminished. Medication Overuse Headaches are one of the worst headache types in destroying quality of Life.

The Neuromuscular Dental Institute (Institute for the Study of Neuromuscular Dentistry) is my answer to this disaster. This wll be in addition to the current course I give on Sleep Apnea Treatment with oral appliance (Dental Sleep Medicine) as well as coverage of nerve blocks including the SPG block. The SPG or Sphenopalatine Ganglion Block can be incredibly effective in preventing and eliminating migraines. Ideally patients can learn to utilze and self administer SPG blocks to prevent or Amelliorate migraine headaches early in their course. It is simple, inexpensive and frequently incredibly effective.

The Alliance of TMD organizations (I am the ICCMO representative to the TMD Alliance) is working to prevent patients from being denied care that will mprove their overall quality of life and subsequently result in enormous long term savings in costs and expenses associates with chronic headaches and migraines.

The way TMJ, TMD and Neuromuscular Dentistry is dealt with by insurance companies is an example of Discrimination against women since the vast majority of patients with headaches, migraines and TM Joint disorders are female.

Methods : A systematic review and qualitative synthesis of studies of patients/subjects with CDH that included CM, occurring on at least 15 days per month. Main findings: Thirty-four studies were included for review (25 studies of patients and nine of subjects from the general population). CDH and CDH with medication overuse headache (MOH) were consistently associated with a lower QoL compared to control or episodic headache (EH) and CDH without MOH. CDH was consistently associated with greater disability and productivity loss, more consultations, more or longer hospitalizations and higher direct costs than EH. Data were not amenable to statistical pooling. Principal conclusions : The findings of this review underline the detriment to QoL and the disabling nature of CDH, and in particular CM and CDH with MOH, and negative impact on workplace productivity compared to other types of headache.

ARURICULAR NERVE STIMULATION FOR TREATING MIGRAINE vs NEUROMUSCULAR DENTISTRY AND DIAGNOSTIC ORTHOTIC TREATMENT

An article in Headache "Implanted auriculotemporal nerve stimulator for the treatment of refractory chronic migraine' (see abstract below) reports on a case of refractory headache with phonopobia treated by implanted peripheral nerve stimulators. Treatment reduced but did not eliminate the patients pain. The authors chose to only treat only a single branch of the mandibular nerve and did not address the entire Trigeminal Nervous system. A second article in Headache "Suboccipital nerve blocks for suppression of chronic migraine: safety, efficacy, and predictors of outcome." discusses the use of occipital nerve blocks in treating migraines. The correction of forward head position thru neuromuscular dentistry can frequently eliminate the need for these blocks.

The ULF-TENS utilized by Neuromuscular Dentistry stimulates all three divisions of the Trigeminal nerve. The anti-dromic stimulation also causes plsing and relaxation of the masticatory muscles. A diagnostic orthotic is used to continually reduce the noxious input to the Trigeminal system which is implicated in almost all migraines and most headaches. The trigeminal nerve is often referred to as the dentist's nerve. Neuromuscular dentists often utilize occipital nerve blocks during treatment but can also utilize ULF TENS of he XI cranial nerve, the accessory nerve to eliminate or reduce the need for these blocks.

According to Wikipedia ""The auriculotemporal nerve is a branch of the mandibular nerve that runs with the superficial temporal artery and vein, and provides sensory innervation to various regions on the side of the head" The orgin of the nerve "arises as two roots from the posterior division of the mandibular nerve (The mandibular nerve is a branch of the trigeminal nerve). These roots encircle the middle meningeal artery (a branch of the mandibular part of the maxillary artery, which is in turn a terminal branch of the external carotid artery). The roots then converge to form a single nerve."
The clinical significance according to Wikipedia is "This nerve as it courses posteriorly to the condylar head, is frequently injured in temporomandibular joint surgery, causing an ipsilateral parasthesia of the auricle and skin surrounding the ear. Actually, it is the main nerve that supplies the TMJ, along with branches of the masseteric nerve and the deep temporal."

This does seem to explain one more reason why Neuromuscular Dentistry is so successful at long term prevention, elimination and treatment of of migraines and other headaches. TMJ disorders are frequently called "The Great Imposter" .

The treatment of migraines with peripheral nerve stimulators is an excellent idea but only after a trial of a Neuromuscular Orthotic has not proven successful. Neuromuscular Dentistry leads to healing of the entire trigeminal nervous system as well as correcting cervical and orthopedic problems that interfere with complete relief.

The summary of this article notes the occipital nerve and trigeminal nerve connections. These explain why occipital migraines and cervical pain are relieved thru neuromuscular dental treatment when it is not explained by cervical orthopedic corrections.

Abstract
OBJECTIVE: To report a case of improved pain control and function in a patient with chronic migraine after treatment with auriculotemporal nerve stimulation. METHODS: The patient is a 52-year-old woman with refractory pain in the bilateral temporal distribution and marked phonophobia as a result of chronic migraine. RESULTS: After a successful trial period, the patient underwent implantation of bilateral peripheral nerve stimulators targeting the auriculotemporal nerves. At 16 months of follow up, her average pain intensity declined from 8-9/10 on the numeric rating scale to 5/10. Her function improved as assessed by the Migraine Disability Assessment, from total disability (grade IV) to mild disability (grade II). Her phonophobia became far less debilitating. CONCLUSION: Auriculotemporal nerve stimulation may be useful tool in the treatment of refractory pain in the temporal distribution due to chronic migraine.

"Origin
The auriculotemporal nerve arises as two roots from the posterior division of the mandibular nerve (The mandibular nerve is a branch of the trigeminal nerve). These roots encircle the middle meningeal artery (a branch of the mandibular part of the maxillary artery, which is in turn a terminal branch of the external carotid artery). The roots then converge to form a single nerve.

Course
The auriculotemporal nerve passes laterally to the neck of the mandible, gives off parotid branches and then turns superiorly, posterior to its head and moving anteriorly, gives off anterior branches to the auricle. It then crosses over the root of the zygomatic process of the temporal bone, deep to the superficial temporal artery

Innervation
The somatosensory root (superior) originates from branches of the mandibular nerve, which pass through the otic ganglion without synapsing. Then they form the somatosensory (superior) root of the auriculotemporal nerve. The two roots re-unite and shortly after the branching of secretomotor fibers to the parotid gland (parotid branches) the auriculotemporal nerve comprises exclusively somatosensory fibers, which ascend to the superficial temporal region. Supplies the auricle, external acoustic meatus, outer side of the tympanic membrane and the skin in the temporal region (superficial temporal branches). It also carries a few articular branches which go on to supply the temporomandibular joint.
The parasympathetic root (inferior) carries postganglionic fibers to the parotid gland. These parasympathetic, preganglionic secretomotor fibers originate from the glossopharyngeal nerve (CN IX) as one of its branches, the lesser petrosal nerve. This nerve synapses in the otic ganglion and its postganglionic fibers form the inferior, parasympathetic root of the auriculotemporal nerve. The two roots re-unite and shortly after the "united" auriculotemporal branch gives off parotid branches, which serve as secretomotor fibers for the parotid gland.

Clinical significance

This nerve as it courses posteriorly to the condylar head, is frequently injured in temporomandibular joint surgery, causing an ipsilateral parasthesia of the auricle and skin surrounding the ear. Actually, it is the main nerve that supplies the TMJ, along with branches of the masseteric nerve and the deep temporal.
After a parotidectomy, the nerves from the Auriculotemporal Nerve that previously innervated the parotid gland can reattach to the sweat glands in the same region. The result is sweating along the cheek with the consumption of foods (Frey's syndrome). Treatment involves the application of an antiperspirant or glycopyrrolate to the cheek, Jacobsen's neurectomy along the middle ear promontory, and lifting of the skin flap with the placement of a tissue barrier (harvested or cadaveric) to interrupt the misguided innervation of the sweat glands.
Pain related to a condition call parotiditis, or commonly referred to as " the mumps" will be carried by the auriculotemporal nerve."

Suboccipital nerve blocks for suppression of chronic migraine: safety, efficacy, and predictors of outcome.

Abstract
BACKGROUND: Approximately 1 in 50 Americans is afflicted by chronic migraine (CM). Many patients with CM describe cervicogenic headache. Options for treating CM effectively are at present quite limited. OBJECTIVE: To determine the safety and efficacy of occipital nerve blocks (ONBs) used to treat cervicogenic chronic migraine (CCM) and to identify variables predictive of a positive treatment response. METHODS: Using a uniform dose and injection paradigm, we performed ONBs consecutively on a series of patients presenting with CCM. Patients were stratified according to specific findings found to be present or absent on physical examination. A positive treatment outcome was defined as a 50% or greater reduction in headache days per month over the 30 days following treatment relative to the 30-day pre-treatment baseline. We used a 5-point Likert scale as one of the secondary outcome variables. RESULTS: We treated 150 consecutive patients with unilateral (37) or bilateral (113) ONBs. At the 1-month follow-up visit 78 (52%) exhibited evidence of a positive treatment response according to the primary outcome variable, and 90 (60%) reported their headache disorder to be "better" (44; 29%) or "much better" (46; 30%). A total of 8 (5%) patients reported adverse events within the ensuing 72 hours, and 3 (2%) experienced adverse events that reversed spontaneously but required emergent evaluation and management. CONCLUSION: For suppression of CCM, ONBs may offer an attractive alternative to orally administered prophylactic therapy.

In the most simple terms the way Neuromuscular Dentistry works is change input and output from our brains and central nervous system to muscles, joints, and nervous system end organs in the Trigeminal Nervous System. Our brains are similar to computers. GARBAGE IN- GARBAGE OUT explains in computer lingo how bad input leads to bad output. The brain is basically a biological computer and GARBAGE IN- GARBAGE OUT holds true when it comes to our brains.

Input to the brain comes from two sources, input from the spinal column which accounts for 20% of the total input to the brain and input from 12 pairs of cranial nerves that accounts for 80% of brain input. The cranial nerves are responsible for sight, smell, taste, vision, hearing, proprioception and control of the autonomic nervous system.

The trigeminal nerve accounts for approximately 70% of the input to the brain from the 12 cranial nerves or more than half of total brain input. The Trigeminal Nerve is also known as the Dentist's Nerve. It goes to the teeth, jaw joints, jaw muscles, the periodontal ligaments of the teeth, the muscle that tenses the eardrum, the muscle that opens and closes the eustacian tubes, that innervates the lining of the sinuses and nasal mucosa. It also controls the blood flow to the anterior 2/3 of the meninges of the brain. When we smell menthol that is another trigeminal nerve function which may be why Vicks Vapor Rub works for many pains.

The trigeminal nerve also has a enormous autonomic component and is a chief cause of central sensitization. Central Sensitization is a primary aspect of most headaches and migraines, facial pains, fibromyalgia and almost all other chronic pain syndromes.

GARBAGE IN - GARBAGE OUT takes on new meaning when we are talking about the majority of input to the brain. Neuromuscular Dentistry turns bad data our brain receives into good data. Central Sensitization can turn good input into bad output. Examples are Hyperesthesia where there is an over-reaction to pain stimuli and Allodynia where non-painful input is received as Nociceptive of Pain impulses. Why do patients suffer for years if there is a treatment that can so drastically improve their lives?

Do problems addressed Neuromuscular Dentistry actually affect health and medical costs?

Cranio, The Journal for CranioMandibular Practice published two articles that answer that question. The studies by Shimshak et al showed that patients with TMJ disorders had a 300% increase in medical utilization in all fields of medicine except obstetrics. In other words, aside from not getting pregnant these patients utilize three times the average in medical expenses. Treatment of Temporomandibular disorders and the neuromuscular pathology that cause them can drastically improve patients lives and possibly drastically decrease medical expenses.

The NHLBI report discusses how respiratory disorders related to TMJ disorders can effect many body systems. Dental Sleep Medicine is an extremely effective approach to treating sleep apnea. It is more effective overall than surgery. CPAP is still considered the Gold Standard of treatment for Sleep Apnea but has horrendous issues with patients compliance.

Poor compliance means it works well if used but most patients do not use it. CPAP that is not used is worthless and dangerous because the patient remains untreated. A recent study showed that 60% of patients did not use their CPAP. Other patients refuse to even have a sleep test because they do not want to use CPAP. One study cited at the Trucking and Sleep Apnea conference presented by the American Sleep Apnea Association showed only 5% of truckers using their CPAP. This is a frightening fact considering than patients with untreated sleep apnea have a six-fold increase in motor vehicle accidents. Patients overwhelmingly prefer oral appliances to CPAP when they offered a choice but most patients are never given a choice. That would be understandable if CPAP compliance wasn't an issue. Studies of patients who do use their CPAP show that they average only 4-5 hours a night of use not 7-7 1/2 that is ideal.

Patients with untreated sleep apnea have up to a six-fold increase of risk of heart attacks and strokes which usually occur in the early morning hours. Most CPAP users have already stopped utilizing their CPAP during the early morning hours when the risk is greatest.

Treatment and prevention sleep apnea with oral appliances is now well accepted but is still fighting for its proper place in medicine. In a few years oral appliances will probably account for a vast majority of treatment of mild to moderate sleep apnea.

Morning headaches have two primary causes, TMJ disorders and Sleep Apnea. The NHLBI says Sleep Apnea is a TMJ disorder. There is an FDA approved appliance for preventing migraines the the NTI-TSS appliance.

The Aqualizer appliance, invented by Dr Martin Lerman is an a simple inexpensive appliance that can produce incredible success but does not offer permanent correction. IAn Aqualizer was used to keep M free of pain between her first and second appointment until her diagnostic neuromuscular orthotic was delivered.

Neuromuscular Dental treatment starts with a Diagnostic Orthotic. When treatment effectiveness is assured patients can proceed with long term phase 2 treatment of a permanent removable orthotic, orthodontic correction or a Neuromuscular Reconstruction. New Developments in Migraine Treatment and how they relate to Neuromuscular Dentistry and Posturology

A recent article in Current Opinions in Neurology reviewed Migraine treatments. While it mentioned the use of medications such as Botox (onabotulinum toxin type A), triptans , Topiramate, gabapentin, petasites and tizanidine and a new nasal form of dihydroergotamine it also discussed risk factors associated with Temporo-mandibular (TMJ, TMD) disorders, sleep apnea and treatments like occipital nerve stimulators.

These are important aspects of Neuromuscular Dentistry which utilizes ultra-low frequency TENS to stimulate the Cranial Nerves V, VII and XI. This would create effects on occipital nerves as well. The use of Botox on cranial muscles shows that they are responsible for many migraines. Reducing pathological muscle activity is relatively easy utilizing a neuromuscular diagnostic orthotic. The occipital muscles respond to postural changes in the head position that have been well described by the Quadrant Theorem of Guzay. NUCCA and A/O (atlas Orthogonal) chiropractic are both extremely effective with chronic daily headaches and migraines however the adjustments do not hold without neuromuscular correction of the jaw position.

The use of medications to treat migraines is essential for some patients but the same therapeutic changes in brain chemistry can be accomplished for many patients by changing neural input to the brain via the Trigeminal nerve which (dentist's nerve) which also controls vascular headaches thru control of blood flow to the anterior two thirds of the meninges. The primary control of vascular flow to the brain is always affected by the health of the stomatognathic system. The blood flow to the brain from the internal carotid is also indirectly affected by jaw position and its effects on head posture and carotid blood flow.

The blood flow to the brain is provided by "The circle of Willis (also called the cerebral arterial circle, arterial circle of Willis or Willis Polygon) is a circle of arteries that supply blood to the brain. It is named after Thomas Willis (1621–1673), an English physician" (from Wikipedia) The arteries are "The circle of Willis comprises the following arteries:[2]
Anterior cerebral artery (left and right)
Anterior communicating artery
Internal carotid artery (left and right)
Posterior cerebral artery (left and right)
Posterior communicating artery (left and right)" and also the basilar artery and middle cerebral arteries, though they supply the brain, are not considered part of the circle of Willis. All of these are directly controlled by the Trigeminal Nerves or indirectly affected by head posture.

Neurologists agree that nearly all chronic daily headaches and migraines are controlled to a large extent by the Trigeminal Nerve. Correction of pathology in the oral systems changes the input to the brain and corrects autonomic functioning.

This study (below) qualifies TMD as risk factors but does not delve into how EMG spectral analysis of masticatory muscles could be used to define in a quantitative manner risks of migraines and chronic daily headaches.

Posturology may be considered a new field that combines and correlates many different specialties.

Posture is nowadays a challenge for scientific society as the discipline-oriented-approach of medicine organization does not comply with the functional-model of posture: a motor function implementation requires interdisciplinary perspective to be thoroughly evaluated and, so far, medicine does not provide a single figure with multi discipline skill.
Given the absence of scientific society interest in posture and the presence of obscure posturologist professionals, posturology is way far from from yielding scientific results, moreover, the current lack of scientific knowledge of posture and the growing ascertainment of relationship between diffused social deseases and the postural disorder, makes posturology research demand much more urgent than posturology clinic demand.
Posturology specialization, at present, is not scientifically aknowledged, but, if appropriately conceived, not only might overcome the lack of overview of the current scientific discipline-model approach but is the only way to provide the required functional-model approach to scientific research." Abstract
PURPOSE OF REVIEW: Chronic migraine is a common cause of chronic daily headache, which is often refractory to standard treatment. New research has increased our understanding of this disorder and its treatment. This review focuses on recent clinical trials and advances in our understanding of migraine pathophysiology. RECENT FINDINGS: Migraine research has traditionally focused on the more common episodic form of the disorder, but recent clinical trials have started to focus on chronic migraine or chronic daily headache. Topiramate, onabotulinum toxin type A, gabapentin, petasites and tizanidine are among the agents that appear to be effective in the treatment of chronic migraine. New acute medications including an inhaled form of dihydroergotamine will soon be available and neuromodulatory procedures such as occipital nerve stimulation may be effective for the most disabled patients. In the past few years, other studies have shed light on potential risk factors for chronic migraine such as medication-overuse headache, temporomandibular disorders, obstructive sleep apnea and obesity. SUMMARY: This review explains advances in the treatment of chronic migraine, a common disorder seen in neurological practice. These new advances in preventive treatment and a better understanding of its risk factors will allow clinicians to better identify individuals at greatest risk and prevent the development of chronic migraine.

HEADACHES, MIGRAINES, FIBROMYALGIA AND TMJ DISORDERS

There is an intimate cooection between TMD and Fibromyalgia and Tension-type and Muscle contraction headaches. These can also serve as triggers for many types of migraines and chronic daily headaches.

The connection is the Trigeminal Nerve that controls blood flow to the brain and contributes over half of all input to the brain an central nervous system from the body.

It is generally recognized by neuroscientists and neurologists that almost all headaches and migraines are primarily trigeminal in nature or influenced by the trigeminal nerve.

Neuromuscular dentistry may be the best prevention for these problems that frequently relate to sleep disorders as well.

Neuromuscular dentistry my be the ideal method to improve the your quality of lyour life without excessive medication. Both chiropractic and osteopathic medicine depend on the jaw for stability of the spine and holistic and or alternative health care methods like massage have longer lasting results when postural correction and neurofeedback from the trigemnal nerve are use as a stabilizing force rather than a continuing irritation that prevents healing and interferes with quality of life.

IMPROVING THE QUALITY OF LIFE WITH TMD TREATMENT. NEW ARTICLE IN ACTA ODONTOL SCAND.

IMPROVEMENT IN QUALITY OF LIFE WITH TMD TREATMENT HAS RECENTLY BEEN PUBLISHED. THIS STUDY USED EVIDENCED BASED ARTICLES FROM Medline and Cochrane Library databases. This severely limited the number of studies considered and eliminates publications of exciting clinical work and case reports. This type of search tends toward bias toward drug therapy.

The study showed almost universal improvement in the quality of life with TMD treatment. The twelve papers reviewed showed that the more symptoms and the worse the condition was to begin with the greater the improvement in the quality of life. These results are unmatched in most of medicine where even a 50% improverment is touted. Men and women appeared to improve equally.

The study concluded that: "The reviewed studies convincingly demonstrated that OHRQoL (quality of life) was negatively affected among TMD patients. this coincides with other known materials including Shimshak et al who published in Cranio Journal a 300% increase in medical spending in all medical fields.

OBJECTIVE: Oral health-related quality of life (OHRQoL) is considered an important aspect of different oral conditions. It has also gained increased attention in temporomandibular disorders (TMDs) in recent years. The purpose of this study was to systematically review the literature on OHRQoL and TMDs. MATERIAL AND METHODS: A systematic search of the dental literature was performed using the Medline and Cochrane Library databases, supplemented by a hand search. Various combinations of search terms related to OHRQoL and TMDs were used. Among numerous titles found in Medline, abstracts and eventually full papers of potential interest were reviewed. Twelve papers fulfilled the inclusion criteria and were included in the review. RESULTS: Most studies used the Oral Health Impact Profile, an instrument with good psychometric properties, for evaluation. All articles described a substantial impact on OHRQoL in TMD patients. Only a small proportion of all patients, a few percent, reported no impact at all. The difference between men and women was small and not significant. The impact appeared to be more pronounced in patients with more signs and symptoms. The perceived impact of pain on OHRQoL seems to be substantial. Two studies found that the impact increased with age among TMD patients. CONCLUSIONS: The reviewed studies convincingly demonstrated that OHRQoL was negatively affected among TMD patients.
